Very Newbie Basic Question

Hello all, I've only bee using software for a few days now and I am trying a few different simple projects. The one I am trying to do now, is to have a simple input for from a customer to send me a few fields worth of data and a picture of equipment that they would like to dispose of. I had it working initially and was able to see the image in the list view, but it has stopped working and I am not certain what I did to break the image field?
I am wanting to store the data/image on the SQL server in the database.

Anyone able to point me in the right direction, or help with some basic info. I would be very appreciative.
